OCR description
The covered entity (CE), NVW Newco, LLC, reported that an employee emailed the protected health information (PHI) of 3,232 individuals to unauthorized recipients. The PHI involved included names and treatment information. The CE notified HHS and affected individuals. In response to the breach, the CE revised its policies and procedures and trained or retrained workforce members.
