OCR description
The covered entity (CE), Blue Shield of California, reported that due to a configuration error, the protected health information (PHI) of 624 individuals was viewable by others via the Internet. The PHI involved included medications and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE provided free credit monitoring to affected individuals.
