Scott County, Iowa

The covered entity (CE), Scott County, Iowa, reported that an employee was the target of an email phishing scheme that compromised the protected health information (PHI) of approximately 4,336 individuals. The PHI involved names, addresses, dates of birth, social security and drivers’ license numbers, diagnoses/conditions, lab results, medications, and other treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE enforced its multi-factor authentication, increased its workforce security awareness training, and increased its threat protections to better protect its electronic PHI. OCR provided the CE with technical assistance.
