OCR description
The covered entity (CE), PACE Greater New Orleans, reported that an employee emailed the protected health information (PHI) of 715 individuals to her personal email account. The PHI involved included names, addresses, dates of birth, diagnoses/conditions, lab results, medications, and treatment information. The CE notified HHS, the affected individuals and the media. In response to the breach, the CE and implemented additional administrative, technical, and security safeguards, and retrained its workforce members to better protect its PHI.
