CHC Solutions, Inc.

CHC Solutions, Inc., the covered entity (CE), reported that an employee was the target of an email phishing scheme that compromised the protected health information (PHI) of 794 individuals. The PHI involved included names, addresses, dates of birth, health insurance information and diagnosis codes. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative and technical safeguards to better protect its sensitive data including security awareness training for all of its staff.
