OCR description
The covered entity (CE), Lake Psychological Services, LLC, reported that it experienced a cyberattack that compromised the protected health information (PHI) of 987 individuals. The PHI involved included diagnoses/conditions and other treatment information. The CE notified HHS and the affected individuals of the breach. In response to the breach, the CE implemented additional technical safeguards to better protect its PHI.
