OCR description
The covered entity (CE), Heart to Heart Hospice Holdings, LLC, reported that it experienced a cybersecurity incident that affected the protected health information (PHI) of 19,034 individuals. The PHI included demographic information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI.
