OCR description
The covered entity (CE), The Pavilion at HealthPark, LLC dba Park Royal Hospital reported that it was the subject of a data breach that affected the protected health information (PHI) of 9,349 individuals. The PHI involved included names, admission dates, provider information, and status as a patient at Park Royal. The CE notified HHS and affected individuals. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards.
