OCR description
The covered entity (CE), Pineland Community Service Board, reported that it experienced a ransomware attack that that affected the protected health information (PHI) of 19,010 individuals. The PHI involved included names, social security numbers, addresses, birthdates, diagnoses/conditions, medications, and other treatment information. The CE notified HHS, the affected individuals, the media, and posted substitute notice. In response to the breach, the CE provided complimentary credit monitoring services to the affected individuals and implemented additional physical, technical, and security safeguards to better protect its PHI.
