OCR description
William F Rinehart DMD, the covered entity (CE), reported that it experienced a ransomware attack that compromised the protected health information (PHI) of 25,000 individuals. The type of PHI affected included names, addresses, birthdates, driver’s license and Social Security numbers, medications, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach the CE implemented additional administrative and technical safeguards.
