OCR description
Concord Orthopaedics, the covered entity (CE) reported that it experienced a hacking attack that compromised the protected health information (PHI) of 72,815 individuals. The PHI involved clinical and demographic information. The CE notified HHS, affected individuals, and the media and implemented additional administrative, technical, and security safeguards.
