OCR description
The covered entity (CE), Western Wayne Family Physicians, reported that it experienced a hacking attack that compromised the protected health information (PHI) of 62,000 individuals. The PHI involved included clinical information. The CE notified the affected individuals. In response to the breach, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI.
