OCR description
Sentara Health, the covered entity (CE) reported that the protected health information (PHI) of 1,620 patients may have been accessed by an unauthorized individual impersonating an employee. The PHI involved included names, addresses, dates of birth, Social Security numbers, diagnoses/conditions, medications, and other treatment information. The CE notified HHS, impacted individuals, and the media. In its mitigation efforts, the CE implemented additional administrative and security safeguards to better protect its sensitive data.
