OCR description
The business associate (BA), COSE Healthcloud Solutions, reported that an employee was the target of an email phishing scheme that affected the protected heath information (PHI) of 2,606 individuals. The PHI involved included dates of birth, medical record numbers, health insurance information and medical treatment information. The BA notified HHS, affected covered entities, and affected individuals. In response to the breach, the BA conducted employee training and implemented additional administrative, technical, and security safeguards.
