OCR description
The covered entity (CE), California Correctional Health Care Services, reported that a workforce member sent an unencrypted email containing the protected health information (PHI) of 4,105 individuals to the wrong recipients. The PHI involved included names, diagnoses, medications, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE retrained workforce members to better protect PHI.
