OCR description
The covered entity (CE), Erlanger Health, reported that its business associate (BA) experienced a ransomware incident that affected the protected health information (PHI) of 3,193 individuals. The PHI involved included names, addresses, Social Security numbers, dates of birth, and other treatment information. The CE notified HHS, the affected individuals, and the media, and posted substitute notice on its website. In response to the breach, the CE offered free credit monitoring to affected individuals.
