OCR description
City of Long Beach, CA, the covered entity (CE), reported that it experienced a hacking incident that affected the protected health information (PHI) of 258,191 individuals. The PHI included demographic, clinical, and financial information. The CE provided timely breach notification to the HHS Secretary, affected individuals, and media. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards. Credit monitoring was also provided.
