OCR description
The covered entity (CE), WPM Pathology Laboratory, Chartered, reported that it experienced a hacking incident that affected the protected health information (PHI) of 5,694 individuals. The PHI involved included names, dates of birth, addresses, drivers’ license and Social Security numbers, and diagnoses and conditions. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional administrative, technical, and security safeguards.
