OCR description
Rheumatology Associates of Baltimore, the covered entity (CE) reported that its business associate (BA) experienced a cybersecurity incident that affected the protected health information (PHI) of 28,968 individuals. The PHI included names, addresses, dates of birth, and Social Security numbers. The BA notified impacted individuals and the media and the CE notified HHS of the breach. In its mitigation efforts, the CE and BA implemented additional administrative and technical safeguards to better protect sensitive data.
