OCR description
Palo Verde Hospital, a covered entity (CE), reported that it experienced a ransomware attack affecting the electronic protected health information (ePHI) of 42,962 individuals. The ePHI involved included individuals’ addresses, ZIP codes, dates of birth, names, Social Security Numbers, other financial information, diagnoses, conditions, lab results, medications, and other treatment information. The CE notified individuals, the media, and HHS. In response to the breach, the CE implemented several corrective actions, including resetting all passwords, multifactor authentication, and additional technical safeguards.
