OCR description
The covered entity (CE), Family Christian Health Center, reported that an unauthorized actor may have accessed its network, affecting the protected health information (PHI) of 48,151 individuals. The PHI involved included clinical and demographic information. The CE notified HHS and the affected individuals. In response to the breach, the CE provided complimentary credit monitoring services to the affected individuals, and implemented additional administrative, technical, and security safeguards to better protect its PHI.
