The covered entity (CE), Anesthesia Associates of Morristown, P.A., reported that its business associated (BA) improperly disposed of documents containing the protected health information (PHI) of 34,675 individuals. The PHI involved clinical, demographic, and financial information. The CE notified HHS, the affected individuals, the media, and provided substitute notice.
