OCR description
The covered entity (CE), Doctors Hospital at Renaissance, Ltd., reported that an unauthorized party gained access to its network environment, accessing and exfiltrating the protected health information (PHI) of 34, 060 individuals. The PHI involved included demographic information. The CE notified HHS and the affected individual. In response to the breach, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI.
