OCR description
HopeHealth, Inc., the covered entity (CE), reported that it experienced a hacking attack that compromised the protected health information (PHI) of 5,823 individuals. The PHI involved clinical, demographic, and financial information. In response to the incident and OCR’s investigation, the CE implemented additional technical safeguards.
