OCR description
The covered entity (CE), Swedish Edmonds Hospital, reported that it experienced a ransomware incident that affected the protected health information (PHI) of 886 individuals. The PHI involved included names, addresses, Social Security numbers, dates of birth, financial information, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE offered free credit monitoring and implemented additional technical safeguards.
