OCR description
The covered entity (CE), Cahaba Center for Mental Health, reported that suspicious activity in an employee account that affected the protected health information (PHI) of 15,085 individuals. The PHI clinical, demographic, and financial information. The CE notified HHS and the affected individuals. In response to the breach, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI.
