OCR description
The covered entity (CE), Erlanger Health, reported that its business associate (BA) experienced a cybersecurity event that affected the protected health information (PHI) of 3,371 individuals. The PHI involved included names, birthdates, medical record numbers, dates of service, and limited medical information associated with sleep studies. The CE notified HHS, affected individuals and the media about the breach and provided substitute notice on its website. OCR provided technical assistance regarding the HIPAA Rules.
