OCR description
The Business Associate (BA), Ocuco, reported that it experienced a hacking incident that affected the protected health information (PHI) of 240,961 individuals. The PHI involved included names, dates of birth, addresses, drivers’ license numbers, Social Security numbers, claims and financial information, diagnoses/conditions, lab results, medications, and other treatment information. In response to the breach, the BA provided complimentary credit monitoring services to the affected individuals, and implemented additional administrative, technical, and security safeguards to better protect its PHI.
