OCR description
The covered entity (CE), Jupiter Family Medicine PC, reported that several employees impermissible accessed and removed the protected health information (PHI) of 3,000 individuals. The PHI involved included demographic information. The CE notified HHS, the affected individuals and the media. In response to the breach, the CE implemented additional administrative, technical, and security safeguards, and retrained workforce members to better protect its PHI.
