OCR description
The covered entity (CE), TRG, LLC, reported that its business associate (BA) experienced a ransomware incident that affected the protected health information (PHI) of 142,004 individuals. The PHI involved clinical, demographic, and financial information. The CE notified HHS, affected individuals, and the media. The BA notified law enforcement. In response to the breach, the CE offered free credit monitoring services.
