OCR description
The covered entity (CE), Cancer Care Center of North Florida-Lake Butler reported that multiple employees of its business associate (BA) were the targets of an email phishing scheme that compromised the protected health information (PHI) of 976 individuals. The PHI involved demographic, clinical, and financial information. The CE notified HHS and confirmed that its BA notified the affected individuals, and the media.
