OCR description
The covered entity (CE), Golden State Radiation Oncology, reported that multiple employees of its business associate (BA) was the subject of an email phishing scheme that affected the protected health information (PHI) of 2,130 individuals. The PHI involved included clinical and demographic information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE provided complimentary credit monitoring services to the affected individuals and implemented additional administrative, technical, and security safeguards to better protect PHI.
