OCR description
The covered entity (CE), Southwest Urology, reported that employees of its business associate (BA) were the targets of an email phishing scheme that affected the protected health information (PHI) of 7,214 individuals. The PHI involved included clinical and demographic information. The BA notified HHS, affected individuals, and the media on behalf of the CE. In response to the breach, the CE provided complimentary credit monitoring services to the affected individuals, and the CE and BA implemented additional administrative, technical, and security safeguards.
