OCR description
Complete Care Rehab, the covered entity (CE), reported that it detected suspicious activity in its network and its lone server was encrypted by a ransomware group. The protected health information (PHI) of 4,764 individuals was affected. The PHI involved included clinical, demographic, and treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented new administrative and technical safeguards and trained or retrained its workforce.
