OCR description
The covered entity (CE), Oregon Specialty Group, reported that it experienced a hacking incident to its network environment that affected the protected health information (PHI) of 3,337 individuals. The PHI involved clinical, demographic, and financial information. The CE notified HHS, the affected individuals, and the media. In response to the breach, the CE implemented additional technical and security safeguards.
