OCR description
The covered entity (CE), Blue Shield of California, reported that an unauthorized individual gained access to the electronic protected health information (ePHI) of 783 individuals. The ePHI involved included names, addresses, dates of birth, Social Security numbers, and insurance identification information. As a result of OCR’s investigation, the CE implemented additional technical safeguards to better protect sensitive data.
