OCR description
The covered entity (CE), Berkshire Health Systems, Inc., reported that an employee was the target of an email phishing scheme that affected the protected health information (PHI) of 1,421 individuals. The PHI involved included clinical and demographic information. The CE notified HHS and the affected individuals. In its mitigation efforts, the CE sanctioned workforce members involved and retrained its staff on the requirement to protect and secure PHI.
