OCR description
The covered entity (CE), Anderson & Kreiger LLP, reported that its business associate (BA) was the subject of a ransomware attack that affected the protected health information (PHI) of 1,607 individuals. The PHI involved included clinical and demographic information. The CE notified HHS, the affected individuals, the media, and posted substitute notice on their website.
