OCR description
Kathalene Keller Riney NP dba Life in Motion Family Wellness Center. the covered entity (CE), reported that its business associate (BA) impermissibly disclosed the protected health information (PHI) of 3,747 individuals. The PHI involved included names, addresses, birthdates, phone numbers and health insurance information. The CE notified HHS, affected individuals, and the media. In response to the breach and OCR’s investigation, the CE revised its policies and procedures, implemented additional technical safeguards and retrained its workforce members.
