OCR description
The covered entity (CE), Indiana Exceptional Medical Care, reported that the CE disclosed the protected health information (PHI) of 1,850 individuals to an unauthorized individual. The PHI involved included demographic information. The CE notified HHS, the affected individuals and the media. In response to the breach, the CE implemented additional administrative, technical, and security safeguards, and retrained workforce members to better protect PHI.
