OCR description
Sentara Health, the covered entity (CE), reported that an employee mailed the protected health information (PHI) of 696 individuals to the wrong recipients. The PHI involved included names, addresses, and treatment information. The CE notified HHS and the affected individuals. The CE also took corrective actions in response to the breach, implementing additional administrative, technical, and security safeguards to better protect its PHI.
