OCR description
The covered entity (CE), Pennsylvania Hospitalist Group, reported that its business associate (BA) experienced a breach of its network by an unauthorized threat actor. According to the breach report, the breach affected approximately 615 individuals. The protected health information involved demographic and treatment information. The BA reported the breach to HHS for the CE, notified the media, and notified the affected individuals. OCR provided technical assistance to the CE regarding the HIPAA Rules.
