OCR description
The covered entity (CE), The Bortolazzo Group, LLC, reported that its business associate (BA) experienced a breach of its network that affected the protected health information (PHI) of 1,491 individuals. The PHI involved included clinical and demographic information. The BA notified HHS, the affected individuals, and the media of the breach. In response to the breach, the CE implemented additional administrative, technical, and security safeguards to better protect PHI.
