OCR description
The covered entity (CE), Trinity Emergency Physicians, LLC, reported that its business associate (BA) experienced a breach of its network that affected the protected health information (PHI) of 1,491 individuals. The PHI involved included clinical and demographic information. The BA reported the breach to HHS for the CE, notified the media, and notified the affected individuals. In response to the breach, the CE and BA implemented additional administrative, technical, and security safeguards to better protect its PHI.
