OCR description
The business associate (BA), Trusteed Plan Services Corporation, reported that an unauthorized user gained access to BA’s network through its VPN using stolen credentials, affecting the protected health information (PHI) of 7,977 individuals. The PHI involved clinical and demographic information. On behalf of covered entities, the BA notified HHS and affected individuals. In response to the breach, the BA provided complimentary credit monitoring services to the affected individuals, implemented additional administrative, technical, and security safeguards, and retrained workforce members to better protect PHI.
